OrthoServe Custom Orthotics Lab ("OrthoServe," "we," "us," or "our") is a DMEPOS (Durable Medical Equipment, Prosthetics, Orthotics, and Supplies) fabrication laboratory located at 2603 Webster Ave, Bronx, NY 10458. We fabricate custom orthotic and prosthetic devices for licensed healthcare providers, podiatry clinics, orthopedic practices, physical therapists, medical distributors, and individual patients across New York and the tri-state area.
OrthoServe operates as a supplier of medical-grade custom devices. We do not provide medical diagnoses, clinical evaluations, or direct patient care. All devices are fabricated pursuant to valid prescriptions from licensed healthcare professionals.

Custom orthotic and prosthetic devices are regulated medical devices. All orders placed with OrthoServe must be accompanied by a valid prescription from a duly licensed healthcare professional authorized under New York State law, including but not limited to:
OrthoServe reserves the right to withhold fabrication or delivery of any order pending verification of a valid prescription. Submitting false or fraudulent prescription information is a violation of these Terms and may constitute a violation of applicable federal and state law.
OrthoServe fabricates devices in compliance with applicable Medicare and Medicaid DMEPOS supplier standards, L-code billing guidelines, and New York State Department of Health regulations. Healthcare providers and wholesale partners are responsible for ensuring that orders submitted comply with their own applicable billing, licensing, and clinical standards.
OrthoServe does not guarantee insurance reimbursement for any device. Coverage determinations are the sole responsibility of the ordering provider and the patient's insurance carrier.

By applying for a wholesale account, you represent and warrant that your organization holds all required state and federal licenses, NPI registration, and any applicable DMEPOS supplier accreditation. OrthoServe reserves the right to verify credentials, request supporting documentation, and approve or deny any account application at its sole discretion. Account privileges may be suspended or terminated for violation of these Terms.
All orders are subject to acceptance by OrthoServe. We reserve the right to decline or cancel orders at any time for any reason, including but not limited to product unavailability, suspected fraud, or incomplete prescription documentation.
Prices are subject to change without notice. Wholesale pricing is governed by your current account tier and order volume agreement. Payment is due as specified at checkout or per your wholesale account terms. Unpaid balances may result in suspension of account privileges.
Custom orthotics and prosthetics are fabricated to order using your submitted casting data, clinical notes, and prescription specifications. Turnaround times are estimates and are not guaranteed. OrthoServe shall not be liable for delays caused by incomplete order submissions, shipping carrier delays, or circumstances beyond our reasonable control.
Risk of loss or damage passes to the ordering party upon delivery to the shipping carrier. OrthoServe recommends inspecting all packages upon receipt and reporting any shipping damage within 48 hours.
Because all devices are custom-fabricated to individual specifications, returns are not accepted except in cases of verified manufacturing defects.
OrthoServe makes no warranty, express or implied, regarding clinical outcomes. Individual results depend on patient compliance, clinical judgment of the prescribing provider, and factors outside our control.

Before initiating any legal action, you agree to first contact OrthoServe in writing to attempt to resolve the dispute informally. If the dispute cannot be resolved within 30 days, either party may pursue formal legal remedies.
OrthoServe reserves the right to modify these Terms of Use at any time. Material changes will be posted on this page with an updated effective date.
